We do not find accident models that focus solely on personal factors in use in industry today. The first model considering accidents and their context in a systematic way was established in the 1930s. The so-called domino theory in particular has significantly contributed to current theories and practices in safety management. There are several subsequent models that focus on the sequence of causes in a way that is similar to the domino theory. Since the 1960s, the field of quality management has been an important contributor to the development of safety management as it is practised today.

Accidents were viewed as phenomena beyond human control. Since the mid-nineteenth century, industrialisation has involved the harnessing of various energy sources such as coal, hydrocarbons, and hydropower for production purposes. Mechanisation and automation, together with new principles for management and organisation, contributed to the separation of individuals from the direct control of the energies used in production. At the same time, the energy flow increased – as did the potential for accidents with severe consequences.

Reporting accidents to the authorities fulfils different aims. One is to supply the authorities responsible for the development of safety regulations with the necessary knowledge for prevention. A second aim is to provide input to the programs for supervision of compliance with workplace regulations. 3 Workers’ compensation systems Most countries have compensation systems for work-related disabilities (injury or illness) or premature death (Stellman 1998).
